What this error means
In Tag Manager, a Google Ads conversion tag only fires when its trigger matches and the container is live. If the tag, trigger, conversion linker or IDs are wrong, or a blocker or consent tool stops the Google tag, no conversion is recorded.
Why it happens
1Wrong or missing IDs
The ID is unique per account and the label per conversion action. Auto-created actions have no standalone label.

2The trigger does not match
The tag only fires when one of its triggers matches.
How to tell if this is you: Preview shows the tag as Not fired on the thank-you event.
3No conversion linker
Landing pages need a conversion linker tag so click data is stored.

4Duplicate or legacy tags
Legacy tags, several containers or an iframe can double count or break tracking.

How to fix it
Fix IDs and triggers

- Copy the Conversion ID and Label from the conversion action, using a manual action if needed.
- Pick triggers that match when the conversion happens.
- Use a variable for the value and Transaction ID.
Add the linker and remove duplicates
For: No conversion linker, Duplicate or legacy tags
- Add a conversion linker tag and fire it on all pages.
- Remove legacy tags and extra containers.
Test with Tag Assistant
- Enable the container for debugging, then connect Tag Assistant.
- Disable ad blockers on the pages you test.
- Allow Google tags through your consent tool.
- Test in Preview, then publish.
How long it takes
After a Tag Assistant debug session, verified actions update in Google Ads in about 30 minutes. After other fixes, Google says to allow 24 to 48 hours for the status to update.
How to stop it happening again
- Test every container change in Preview before you publish.
- Use a Transaction ID to avoid duplicate conversions.
- Remove legacy conversion tags when you move to Tag Manager.

In Preview I check whether the trigger fired on the right event and page, and whether blocking or consent settings stopped it. Then I look at the tag setup and variables. The Tags Not Fired list shows which condition failed.
