Usually the Purchase event fires but the value and currency are hardcoded, empty, or pulled from a variable that doesn't exist on the confirmation page. I open the event in the test events tool and look at the parameters it received. If they're blank, the fix is in the tag or the data layer, not in Ads Manager.
